Before creating your donor profile, please read this Data Privacy Consent Form carefully.
The DUGO system respects your right to privacy and is committed to protecting your personal information in accordance with Republic Act No. 10173, otherwise known as the Data Privacy Act of 2012, and its Implementing Rules and Regulations. The Act requires personal information to be processed in accordance with the principles of transparency, legitimate purpose, and proportionality.
PERSONAL INFORMATION TO BE COLLECTED
The DUGO system may collect and process personal information provided during donor registration, eligibility screening, appointment scheduling, blood donation activities, and related services.
The information may include:
- Full name
- Contact information
- Date of birth and age
- Address
- Identification details
- Blood type
- Donation history
- Eligibility and screening information
- Appointment information
- Other information necessary for legitimate blood donation operations
Some information collected during donor screening may qualify as sensitive personal information under Section 13 of Republic Act No. 10173, particularly information relating to an individual's health. Such information shall be processed only for lawful and specified purposes and with appropriate safeguards.
PURPOSE OF PROCESSING
Your personal information will be collected and processed for the following purposes:
- Creating and managing your donor profile.
- Processing donor registration and eligibility screening.
- Scheduling and managing blood donation appointments.
- Maintaining accurate donor and donation records.
- Recording and managing blood donation activities.
- Providing notifications and updates related to blood donation activities.
- Supporting blood donation and blood inventory operations.
- Generating reports and statistics for authorized system personnel.
- Supporting the administration and coordination of blood donation activities.
- Improving the efficiency and delivery of blood donation services.
The information collected shall be limited to what is necessary and relevant to the declared purposes of the DUGO system, consistent with the principle of proportionality under the Data Privacy Act of 2012.
LEGAL BASIS FOR PROCESSING
The processing of personal information shall be performed in accordance with the lawful processing requirements of Republic Act No. 10173.
Where consent is required, your consent shall be freely given, specific, informed, and properly recorded. The processing of sensitive personal information shall also comply with the requirements of Section 13 of the Data Privacy Act of 2012.
DATA STORAGE AND SECURITY
Your personal information shall be stored within the DUGO system and protected through appropriate organizational, physical, and technical security measures.
Access to personal information shall be restricted to authorized personnel who require the information to perform their assigned responsibilities. Unauthorized access, disclosure, alteration, or processing of personal information is prohibited.
Personal information shall be retained only for as long as necessary to fulfill the declared purposes of processing, subject to applicable laws, regulations, and legitimate operational requirements.
DATA SHARING AND DISCLOSURE
Your personal information shall not be disclosed to unauthorized individuals or organizations.
Information may be disclosed or shared only when necessary for legitimate blood donation operations, when authorized by law, when required to fulfill a lawful obligation, or when otherwise permitted under the Data Privacy Act of 2012 and applicable regulations.
Any authorized sharing of personal information shall remain subject to appropriate privacy and security safeguards.
